Orthopedic Consultants Medical Group has implemented a new electronic medical record system in our office. This allows our physicians and clinical staff to locate all of your patient information more quickly, update your records easily, and ultimately make your experience in our office more time efficient and pleasant.

Are you a New/Existing Adult Patient? Completely fill out, and bring with you to your appointment all of the following forms:

• HIPAA
• Financial Statement
• PHI

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon to the right. This will take you to Adobe's web site and enable you to download the Acrobat program to your computer.

Are you a New/Existing Child Patient? Completely fill out, and bring with you to your appointment all of the following forms:

• PHI
• HIPAA
• Financial Statement

Are you a New/Exisiting Workers’ Compensation Patient? Make your first appointment by calling our Worker’s Compensation Coordinator at 800-841-5130 or 818-788-7343 x 229. Next, completely fill out, and bring with you to your appointment all of the following forms:

• PHI
• HIPAA
• WC Questionnaire
